QUARANTINE INSTRUCTIONS
You should not have traveled outside of NJ for 14 days before coming to The Stone Center.
The purpose of this quarantine is to reduce your chance for an exposure to COVID-19 before your procedure at The Stone Center. Start your quarantine immediately after you complete the COVID-19 test and continue until your scheduled procedure date.
YOU MUST MAINTAIN QUARANTINE.
- DO NOT LEAVE THE HOUSE UNTIL YOUR PROCEDURE DATE
- USE YOUR OWN THINGS: Don’t share dishes, drinking glasses, cups, eating utensils, towels or bedding, etc. with other people in your home.
- HAND WASHING: Hand washing before & after eating; after touching surfaces, doorknobs, using the bathroom, etc. HAND WASH 20 SECONDS MINIMUM
- STAY AWAY FROM OTHERS IN YOUR HOUSE: Stay in your room & sleep alone if possible.
- SOCIAL DISTANCING AND WEAR A FACE MASK: If you MUST be around people in your household, keep AT LEAST 6 feet between you and household members and wear a mask. Maintain at least 6 feet distance if you cannot eat alone.
- USE YOUR OWN BATHROOM IF POSSIBLE: if not, it should be cleaned in between uses with the recommended household cleaners. (Clorox products, Lysol products, soap & water, etc.)
- STAY AWAY FROM ANY VISITORS WHO COME TO YOUR HOUSE
- MONITOR YOUR SYMPTOMS: See symptom screen.

Symptom Screen for COVID-19
You should not have traveled outside of NJ for 14 days before coming to The Stone Center.
During your quarantine you must monitor your health daily for any of the following symptoms and take a daily temperature.
SYMPTOM SCREEN
- Temperature – if above 100ᵒ F
- Chills
- Shortness of breath or difficulty breathing
- Sore throat
- Chest Pain
- Malaise, fatigue
- Cough
- Headache
- Body aches, Joint pain
- Loss of smell
- Loss of taste
- Nasal Congestion
- Runny nose
- Nausea/vomiting
- Diarrhea
- Any skin rashes
- Come in contact with anyone who is sick
- Come in contact with anyone who has COVID-19
- Come in contact with anyone who may have COVID-19
